Mount Airy provides a variety of forms and applications to meet your ministry business needs. Some of these documents are available as online forms and others may require downloading to your PC. Viewing these documents may require the Adobe Reader software which is available as a free download at:  Adobe Reader.

If you desire to request the use of the building please make sure the Church calendar is clear on the date desired and fill out a building request form. Please turn it in to the church office or email it to the Church Secretary – Attention Church Administrator, our Office Manager.

If you are planning an activity and want our church family to be informed or participate then please fill out an Activity Request Form. Please turn it in to the church office or email it to the Church Secretary – Attention Church Administrator, our Office Manager.

If your event or a ministry idea will need to be funded, please fill out a Request for Funds Form below. A request form for funds has to be approved by the church Treasurer, or the person responsible for that area of ministry. A list of those who may authorize the form is below. If the funds are a request for reimbursement, they will have to be approved ahead of time. The Treasurer will also be the final approval on all requests for funds, as authorized by the Church Governing Board. For info about the request form process read this file:
